KCI Safety Orientation

1. I am required to report any incident, near miss, property damage, etc. Immediately to my Supervisor or KCI Superintendent

2. In the event of an emergency, I am responsible for calling 911

3. Tobacco use in the building is permitted

4. Prior to each day’s work a Jobsite Safety Analysis (JSA) must be completed/documented, reviewed with the crew, and signed by all employees acknowledging information and training

5. Ground Fault Circuit Interrupters (GFCIs) must be used to protect all construction power?

6. It is okay to use the top two steps of a step ladder, standing or sitting

8. The disciplinary policy may be used to ensure compliance with OSHA, Owner, and Site Specific Safety Requirements; immediate removal from the site can occur for serious or repeat violations

9. Operation of an aerial lift, boom or scissor, requires operator certification, knowledge of potential hazards to the operator, users, public, fellow workers, documented daily inspection and safe operation

10. 100% fall protection is required on this jobsite, but before use of fall protection systems all efforts should be made to engineer out the hazard

11. Personal Fall Arrest Systems must be designed to limit a fall between 2’ and 6’ and prevent contact with a lower level.

12. All scaffolds must be inspected prior to use, be full decked and include a full guardrail system regardless of height

13. Hotwork is any spark or heat producing activity, special protection is required including issuance and work under a Hotwork Permit, Hazard Specific PPE, Public protection, fire protection/extinguishers

14. A face shield over safety glasses/fitted safety glasses or goggles is required for grinding, cutting and other hazards from work generating flying debris

15. Demolition activities presents special hazards are requires a detailed safe work plan submitted, reviewed and approved prior to start of the activity

16. Physical and/or Visual Barricading and/or additional hazard warning signage should be used to warn of hazards from vehicle traffic, heavy machinery, rotating machinery, falling debris, and overhead work or hoisting areas

17. Electronics, music devices, boom boxes are permitted

18. Permits are required for confined space entry and hot work

19. This is a drug and alcohol free site, all employees are required to be actively enrolled in a drug and alcohol prevention program, including initial testing, random, post accident & for cause

20. Dedicated fire watch should monitor the area after heat source is eliminated for how long

21. Everyone is required to attend a weekly tool box talk

24. Hard hats, safety glasses, and work boots must be worn at all times on site unless otherwise identified on the daily JSA

25. Failure to properly use PPE may be grounds for immediate removal from the site

26. It’s ok to carry a gun as long as you let someone know about it

27. Rules for Extension ladders include: stable footing, secured from movement, have non-skid feet, set up at a 4 to 1 ratio, inspected prior to each use, maintain 3 points of contact, 3’ above access area

28. If I show up under the influence of drugs or alcohol the only repercussion is that I will be sent home to sleep it off and allowed to return the next day, no questions asked

29. It is acceptable to wear cut off jeans, ragged clothing and sleeveless shirts

30. Safety is everyone’s responsibility and it is my duty to stop work due to an unsafe act or condition until that condition is resolved

31. Safety glasses or prescription eyewear must meet the requirements of ANSI Z87.1 for impact protection (be labeled accordingly) and equipped with side shields

32. I have been informed and/or shown where to park my vehicle, the locations of restrooms, prohibited areas, fire extinguisher locations, where to find the First Aid kit, the Evacuation Point(s), location of MSDS and designated break areas
