Please have the following SOPs to hand before starting this assessment.
 
The first part of this assessment will focus on the breathalyser. The second part will assess urine sample collection for both POC and lab analysis.

Why is it important to charge the Martel mobile printer prior to a sample collection?:
It makes the printer print faster
The printer uses rechargeable batteries that must be regularly charged to avoid them going flat
The AC adapter DOES NOT provide power to the printer, it just charges the batteries
What MUST you ask the donor prior to taking a breath sample?:
Have they smoked, eaten, chewed gum or used mouthwash in the last 15 minutes?
When was the last time the consumed alcohol?
If “Low Flow” appears on the breathalyser screen, what has happened and how should you proceed?:
The donor has not provided a sufficient amount of breath sample. Reset the device and advise the donor to blow harder for longer
The donor has blown too hard in to the breathalyser. Reset the device and ask the donor to provide a really shallow breath
What is the Rail Standard cut off level for Alcohol Breath Analysis?:
Anything above zero
13ug/100ml (A breath reading of 14 or above would be a FAIL)
If the breath result was 5ug/100ml how would you proceed?:
Just record the result
Call CHG immediately, but a second breath sample is not required as the result is lower than the cut off level
Take a second breath sample and call CHG whilst the donor is still with you
Under which circumstances would a second breath test be required?:
Breath reading result = 0ug/100ml
Breath reading result = 14ug/100ml
Breath reading result = 13ug/100ml
How long should be left between performing each breath sample from the same donor?:
Take the next breath sample immediately

Why MUST you print the result from the Martel mobile printer?:
The donor needs it to show his employer
It is a RISQS requirement that a Home Office Approved breathalyser is used for all Rail Standard tests and an evidential print-out is produced directly from the device
There are 2 copies printed automatically from the Martel mobile printer, where MUST these go?:
Just complete one of them and destroy the other
Complete both of them and give both copies to the donor
Complete both print outs in full, give one to the donor and the other must be returned to Crystal Health attached to the COC/ROR form
If the donor refuses to provide a breath sample, what MUST you do?:
Complete the Non-conformity section of COC/ROR form. Do not proceed with the collection of the urine sample and call CHG immediately
Do nothing, simply proceed and collect the urine sample
If the donor cannot provide a breath sample due to health issues, what MUST you do?:
Inform the donor that this is not acceptable and they must provide a breath sample, otherwise the result will be recorded as a fail
Call Crystal Health, you will be advised that the alcohol can be tested as part of the laboratory test using the urine sample, you will be asked to amend the paperwork
Which information MUST be recorded on both of the result printouts from the Martel mobile printer?:
Donor’s name
Donor’s place of birth
Donor’s signature
Collection Officer’s name
Collection Officer’s signature
The location of the sample collection
Please select the tamper evident and adulteration checks that must be performed prior to the sample collection of urine?:
Remove potential contaminants from the toilet facilities
Mop all floors and wait until dry
If possible, use toilet facilities without windows
Place blue colouring tablets in the cistern (if possible) and the toilet bowl
Apply tamper tape to all taps, the flush mechanism, cistern and window locks
Display the ‘NO ACCESS’ and ‘DO NOT FLUSH TOILET’ signs
The donor MUST have read the following before beginning the sample collection:
The donor advice sheet
The full sample collection procedure (SOP)
Todays newspaper
Please select the accepted forms of photo ID from the following list:
Current Passport
Current photographic Driving Licence
Bus pass
Official employee photographic ID card or current Sentinel card
If no form of photo ID is available, what must you do?:
Threaten the donor with legal action
If possible, ask the employer to verify the donor’s identity and sign the COC/ROR form to confirm this
If the option of a representative verifying the donor’s identity is not available, call CHG immediately
The time-frame associated with declared medication is?:
Just the last 24 hours
14 days (2 weeks)
Over the donor’s lifetime
Specifically for RISQS (Rail standard) testing, the donor MUST also provide:
A doctors note
Their National Insurance number
A solicitors letter
If the donor refuses to provide a sample, what MUST you do?:
Start arguing with them
Don’t do anything and ask them to leave
Call CHG immediately and complete the Non-conformity section of COC/ROR form
It is important to ask the donor to remove any outer clothing such as a coat/jacket and place any valuables in the supplied security bag or locker. But why is this?:
So they are comfortable during the test
To prevent any concealed urine samples being used as a substitute
What MUST you do if the donor is unable to provide a sample?:
Ask the donor to go and buy a 2 litre bottle of water and drink it as quickly as possible
Advise the donor to drink 250ml of water at 20 min intervals, within a maximum wait period of 2 hours
The donor MUST be supervised by an authorised person at all times when waiting to provide the sample
The donor can go for a coffee while waiting to provide a sample
If after 2 hours a sample cannot be provided, call CHG & complete the Non-conformity section of COC/ROR form
Why is it important that the urine sample temperature is within 32 - 38C:
To ensure that the donor is healthy
To ensure that the sample is body temperature and has not been adulterated in any way
Which of the following statements are TRUE?:
Network Rail tests can utilise Instant POC and/or laboratory testing
Using barcodes prevent samples being mixed up at the laboratory & uniquely identify the samples.
Oral fluid (saliva) samples can be used for Network Rail testing
If a Non-negative result is obtained during POC testing the sample MUST also be sent to the laboratory
All candidates can go straight back to work after testing, regardless of the result obtained
CHG MUST be called immediately for any Non-negative results or breath result greater than zero
The donor is signing consent for:
A job to work on the railway
To have their urine sample tested for the required drugs
Their personal information may have to be disclosed to a Medical Review Officer including declared medication
