DEMO: HR Survey

With continual changes in the workplace, we thought it would be a good idea to touch base with employees through an employee survey.  
 
Our goal in this survey is to assess challenging areas, what is working well, and what could be improved upon. The survey focuses on employee/HR issues such as communication, teamwork/engagement, leadership, tools/resources, training/career opportunities and HR programs. 
 
We have contracted with i2i Workplace to conduct the survey and to provide some feedback on the results.  The survey should take less than 30 minutes. 
 
We encourage you to be candid and constructive in this survey. Your responses are CONFIDENTIAL and individual responses will not be included in the report.  We will report only on numeric ratings and general comments and trends in the open-ended comments. 
 
The survey will ask for ratings (scale of 1-5) on various categories, and provides text boxes after each section for open-ended comments. Use this rubric for reference:
1 =  Poor
2 =  Fair
3 =  Good
4 =  Very good
5 =  Excellent
