If no form of photo ID is available, what must you do?:
Threaten the donor with legal action
If available, request that an authorised representative (e.g. Employer or Solicitor) verify the donor’s identity
If the option of a representative verifying the donor’s identity is not available, call Crystal Health immediately
The time-frame associated with declared medication is?:
Last 24 hours
14 days (2 weeks)
Over the donor’s lifetime
If the donor refuses to provide a sample, what MUST you do?:
Start arguing with them
Don’t do anything and ask them to leave
Complete the Non-conformity section of ROR form
Why is it important for you to wear gloves when obtaining the samples?:
For hygiene purposes
It prevents potential contamination of the sample being collected
It makes us look like we know what we are doing
Before obtaining the urine sample, it is important to ask the donor to remove any outer clothing such as a coat/jacket and place any valuables in the supplied security bag or locker. But why is this?:
The valuables are to be kept as payment for the test
To prevent any concealed urine samples being used as a substitute
To keep the donors valuables safe whilst the sample collection takes place
What questions must you ask the donor prior to using the alcohol oral fluid strip?
Have they eaten, smoked, chewed gum or used mouth wash in the last 15 minutes
Have they eaten, smoked, chewed gum or used mouth wash in the last hour
What is their favourite alcoholic beverage
How should you record the answer to the question regarding whether the donor has eaten, smoked, chewed gum or used mouth wash in the last 15 minutes?
On the oral fluid test strip wrapper
On the back of your hand
On the Record of Result form in the applicable boxes and fields in the Alcohol Screen section
Which of the following waiting times are correct for use and activation of the alcohol oral fluid strip?:
Leave the strip under the donor’s tongue for 2 minutes and then 10 seconds to activate
Leave the strip under the donor’s tongue for 10 seconds and then 2 hours to activate
Leave the strip under the donor’s tongue for 10 seconds and then 2 minutes to activate
What MUST you do if the donor is unable to provide a sample?:
Ask the donor to go and buy a 2 litre bottle of water and drink it as quickly as possible
Advise the donor to drink 250ml of water at 20 min intervals, within a maximum wait period of 2 hours
The donor MUST be supervised by an authorised person at all times when waiting to provide the sample
The donor can go for a coffee while waiting to provide a sample
If after 2 hours a sample cannot be provided, call CHG & complete the Non-conformity section of COC form
It is important that the sample temperature is within the stated range because:
To ensure that the donor is healthy
To ensure that the sample is body temperature and has not been adulterated in any way
What must you do if the temperature fails to register?:
Obtain a fresh urine sample using the spare urine cup
Carry on regardless, it’s probably just a dodgy cup
If a second urine sample fails to register on a new cup, the sample may have been adulterated, stop the test and contact Crystal Health immediately
On occasion, additional drugs must be screened that are not part of the standard panel of drugs integrated in to the cup. Please tick the statements below that are correct for this additional procedure:
The additional drug strips are placed in the mouth of the donor
The additional drug strips are dipped in to the urine for 10 seconds to the dip mark, before screwing the lid on and activating the cup
The additional drug strips are air dried by flicking them vigorously in the donor’s face
Once dipped, the additional drug strips are placed on the drip tray and left to activate for approx 2 mins
How do you activate the cup?:
Stir the urine sample with your gloved finger
The test will activate automatically when the urine enters the cup, results can be read by removing the label.
Screw the lid on tight and give the cup a good shake for 5 minutes
What are the time-scales associated with activating the urine drug cup and additional drug strips?
The drug strips can take up to 20 minutes, the cup usually about the same time
You can observe the additional drug strips activating in the drip tray
The cup can take up to 5 minutes before the results can be read, however, this can be checked after a couple of minutes to see if they are ready
You must not reveal the results anytime before 5 minutes on the cup - if you do, an alarm is activated on the label that you peel back to reveal the results thus invalidating the test
The following procedures apply when reading the drug results:
When a line appears on the Control Line (C), this indicates that the particular drug within the cup has been successfully screened
The Test Line (T) has to be completely blank for a non-negative screen result to be recorded
If no Control Line (C) appears, wait another 30 minutes before checking for the line again
If no Control Line (C) appears, you must decant the existing sample in to a new cup and repeat the procedure
A line on both the (C) and (T) indicates that the drug has been screened successfully and the result is negative i.e. no presence of that particular drug indicated
The additional drug strips work in exactly the same way as the cup and the results are read in the same way
Even if the line is very faint on the Test Line (T), this is still read as a negative result
Where are the drug results recorded?:
In the Non conformity section
On the Record of Result form in the DRUG SCREEN: Details and Results section
Results for any additional drug strips can be recorded on a post-it note and stuck on the Result form
Results for any additional drug strips must be recorded next to the corresponding hand written drug name on the Record of Result form in the DRUG SCREEN: Details and Results section
What action must you take if a Non-negative result is indicated from the screen?:
If authorised, the sample must be sent to the laboratory following procedures. You must contact Crystal Health immediately.
Ask the donor if they have indeed taken that drug, if they say ‘no’, then just mark it down as negative and take their word for it
Double check the results from the cup and ensure that you accurately record the Non-negative result against the correct drug name on the Record of Result form in the DRUG SCREEN: Details and Results section
The following procedures apply in concluding the test
Double check that all applicable sections of the COC drug testing form have been completed
Ensure that the donor has provided full consent and relevant details
The Non-Conformity section has been completed if unable to complete the sample collection
Top copy of the Record of Result form to Crystal Health, yellow copy to company (if applicable) and blue copy to the test donor
Dispose of any used alcohol and drug strips and the urine test cup
If authorised from a Non-negative screen result, send the sample(s) to the laboratory on the same day following approved procedures. You must nform the Crystal Health team of this.
All other paperwork, including your invoice, is placed in the Crystal Health return postal envelope and returned on the same day
