Handoffs and Transitions of Care Test

1. Handoffs between healthcare teams create an opportunity for errors and omission of critical information that may lead to serious patient safety consequences.

2. If the TSA is marked “yellow” the electronic medical record captures all relevant information so there is no need for direct interaction and the face-to-face handoff.

3. Documents that are required in the chart include the face sheet, TSA, green sheet, physician MAR and the Register Nurse (RN) to procedure area handoff.

4. On the green sheet, the contact during the procedure should be the name and number of the RN caring for the patient.

5. If the receiving RN is particularly busy on a given day, it is OK for the US to take report on their patient.

6. Healthcare team members performing the handoff do not need to use checklist because they already know the patient’s medical conditions.

7. When the Perianethesia Care Unit (PACU) RN calls the floor to give report they will only have details about the care the patient received in the PACU?

8. The healthcare team can be placed at risk if patient information at handoff is incomplete.

9. Receiving team confirms accepting patient care only when handoff report is complete.

10. Our patients expect us to transfer all relevant clinical information on to the next healthcare team in order to maximize their safety throughout hospitalization.
