If no form of photo ID is available, what must you do?:
Threaten the donor with legal action
If available, request that an authorised representative (e.g. Employer or Solicitor) verify the donor’s identity
If the option of a representative verifying the donor’s identity is not available, call Crystal Health immediately
The time-frame associated with declared medication is?:
Last 24 hours
14 days (2 weeks)
Over the donor’s lifetime
If the donor refuses to provide a sample, what MUST you do?:
Start arguing with them
Don’t do anything and ask them to leave
Complete the Non-conformity section of COC form
Why is it important for you to wear gloves when obtaining the samples?:
For hygiene purposes
It prevents potential contamination of the sample being collected
It makes us look like we know what we are doing
What question must you ask the donor prior to collecting an oral fluid sample?
Have they eaten, smoked, chewed gum or used mouth wash in the last 15 minutes
Have they eaten, smoked, chewed gum or used mouth wash in the last 24 hours
Do they have bad breath
The following sample collection instructions are correct:
Ask the donor to bow their head forward when the swab is in their mouth
The donor should close their mouth when the swab is in place
The cotton pad of the testing stick must be placed under the tongue like a thermometer
The pad must be air dried before placing into the transport tube
Moving the tongue from side to side before collection will help saliva to build up
Ask the donor to chew on the mouth swab for 5 minutes
Raising the head can facilitate saliva collection
If the adequacy indicator fails to activate after 5 minutes ask donor to remove device re-pool saliva then try again
Place the cotton pad directly into the liquid filled SAMPLE tube immediately after collection.
How do you know when you have enough sample for the mouth swab?:
The adequacy indicator on the mouth swab will turn completely blue
Give it two minutes and there should be enough.
The donor will start choking.
Why is it important to place barcodes on both samples?:
It makes the samples look more official.
It provides a robust method of donor identification.
Using barcodes prevent samples being mixed up once at the laboratory.
The following important administration tasks MUST be completed:
Double check that all applicable sections of the drug testing (COC) form have been completed.
Ensure that the donor has provided full consent and relevant details.
The Non-Conformity section has been completed if UNABLE to complete the sample collection.
Only the white part of the drug testing form has been placed with the samples in the laboratory bag
The yellow part of the drug testing form must be given to the organisation ordering the test and the blue copy to the donor
The green part of the drug testing form must be returned to Crystal Health Group.
