Learning Values Self Assessment - Learning Effectiveness

Although I write things in my planner when my teacher tells me to, I don't look at it before I go home or when I am studying.
I often arrive late to class.
Although I know about methods to help me produce good work, such as the criteria and task sheets, I don't use these.
I often find myself getting distracted by other students and/or random things in or out of the room.
I frequently mishear or misinterpret the teacher’s instructions, probably because I wasn't listening.
I could get higher grades, but I don't put in enough effort.
I am not good at being organised.
I often leave my planner/diary in my locker or at home.
Sometimes I forget the right books for class.
I don’t always have a charged iPad/laptop.
I feel very stressed around test and exam time, because I don't organise my time well.
My work is often handed in late, or I am often asking for extensions.
I don't always get my class work complete.
I struggle to meet the success criteria set for the lesson.
I rush assignments and often complete them at the last minute.
When I ask for extensions, I still don't get the work complete or submitted.
I often attempt tasks but don't hand them in because they’re not finished and I don't feel confident submitting them for assessment.
