During the course of a visit, all patients receive an After Visit Summary (AVS). An After Visit Summary (AVS) is a printed report that summarizes such things as vitals, medications, orders, notes, etc. From the encounter or visit. As a patient experience specialist (Front office assistant) you should print the AVS (After Visit Summary) from the follow:
Patient Station
Account Maintenance
DAR
Dashboard
In the event you have written a note on the wrong patient while logged into the periop department you can delete the note by doing the following:
Epic button, Chart Corrections Request , Submit Request
Contact supervisor or manager who has the security to delete the note
Patient Station, Double-Click encounter, mark encounter as erroneous, re-create encounter with correct note.
Open the patient's chart, Click on Notes, Click on the note you want to delete, Click delete, Select a reason, Type an explanation for why you want to delete the note, Click accept
If a patient has died and needs to be marked as deceased in the EMR, complete the following steps:
Open the patient's chart, Click the More Activities tab in the lower left corner, Select FYI from the list, Click "new flag" in the upper left corner, Select a flag type of Marked Deceased, Click Accept in the lower right corner of the box
Reach out to physician to create a decease encounter and document an H&P Note to flag a patient as deceased.
Do Nothing
Manager pages code blue, physician creates deceased encounter, H&P note is created, family notified, closed encounter, Flag patient record as deceased
