How old is your child?

On a daily basis, do you play with your child doing any of the following: sing, laugh, chant, rhyming games, peek-a-boo games, rolling balls, etc.?

On a daily basis does your child play with an iPad, cell phone, computer, or other electronic devices?

On a daily basis, during mealtimes for an infant, do you hold your infant, talking to them, making eye contact, smiling? For toddlers, do you allow them opportunities to feed themselves while talking to them about what they are eating or what is happening next in the daily routine?

On a daily basis, does your infant or toddler have access to play with toys, such as soft blocks, balls, dolls, dishes, rattles, shape sorters, stacking toys, etc.?

On a daily basis, does your child have lots of opportunities to move freely without restriction? (e.g. Young infants are placed on their back to roll and stretch; toddlers have opportunities to explore and discover with adult supervision)

On a daily basis do you play with your child doing any of the following: constructing with blocks, building bricks, working with art materials, such as crayons, paper, paint; playing with play dough, singing, rhyming, chanting; pretending such as dress up, costumes, playing house?

On a daily basis, does your child spend more than one hour combined watching or playing with computers, iPads, TV, video games, or other screen or electronic devices?

Do you use daily routines (e.g. Meal times, time spent in the car or on public transportation) to talk to your child, ask questions and play simple games?

On a daily basis, does your child pretend play with other children or adults using costumes or props, such as dolls, action figures, pots and plates, etc?
