We are grateful for your interest in engaging with the

Healthy Parkinson's Communities™ (HPC) initiative! 

 

This form includes two parts to help us learn more about you:

1. A strengths-based quiz

2. Short answer questions

We love using strengths quizzes to help us self-reflect, develop professionally, and work together more efficiently. Driven by the success of the strengths science as seen in CliftonStrengths Finder, we believe in recognizing the strengths that come from investing knowledge in your natural talents (*Gallup, 2016). That is why we developed this quiz and integrated it into the application process to join the HPC Community Leaders. 

Individuals in our programs have innate talents that we wish to support through adding knowledge and skills to those aptitudes by support through the most fitting program(s) and resources in the HPC initiative.

We’ll use the statements from the quiz and short answers to match you with one of five “Community Leader Superpowers.” Each superpower comes with a title, definition, and example of the kind of work and activities people with that superpower tend to gravitate toward within the HPC initiative.  

Our Community Engagement staff is here to reflect with you and help to match you with roles and opportunities that align with your superpower, strengths, and interests. We’ll use your results as a discussion tool as we meet to learn more about you and place you in our programs.
